need to obtain a copy of the progress notes from the last three
office visits to your physician that pertained to the condition that the
product will treat.
*Generally, you pay 20% of the Medicare-approved
amount after you pay your Medicare Part B deductible for the year ($162 in
2011). Medicare pays the other 80%. The Medicare-approved amount is the lower of
the actual charge for the item or the fee Medicare sets for the item. However,
the amount you pay may vary because Medicare pays for different kinds of durable
medical equipment in different ways. You may be able to rent or buy the

Medicare LCD (Local
Coverage Determination) Information
"For chronic pain, the medical record must
document the location of the pain, the duration of time
the patient has had the pain, and the presumed etiology of the pain. The pain
must have been present for at least three months. Other appropriate
treatment modalities must have been tried and failed, and the medical
record must document what treatment modalities have been used. The presumed
etiology of the pain must be a type that is accepted as responding to TENS
therapy. Examples of conditions for which a TENS unit are not considered to be
medically necessary are (not all-inclusive): headache, visceral abdominal pain,
pelvic pain, and temporomandibular joint (TMJ) pain."
